1.       Registration Final Message

PTAA is under no obligation to grant special requests for your child to be on a specific team, have a specific coach, or be placed on a team with a specific player other than a sibling, or be re-drafted. Any such request must be made to the BASEBALL or SOFTBALL BOARD and requires board approval. Penn Township Athletic Association also reserves the right to use any photograph/videography taken at any event, sponsored by PTAA, without the expressed written permission of those included within the photograph. Photographs may be used in publications or other media material produced, used or contracted by PTAA including but not limited to: newspapers, magazines, television, websites, etc. Any person desiring not to have their photo taken or distributed must contact the PTAA President in writing of their intentions and include a photograph. The photo will be used for identification purposes and will be held in confidence by the PTAA organization.

2.       Association Release and Consent

I / We, the parent or legal guardian of the registered applicant(s), hereby give my/our approval for his/her participation in any or all association activities during the current year. I/We assume all risk and hazards to, during, and from the activities, and do hereby waive, release, absolve, indemnify and agree to hold harmless the "Penn Township Athletic Association" and its sponsors, managers, assistants, and umpires for any claim arising out of any injury to the applicant incurred while participating in any association related activities. Activities include scheduled games, practices, and workouts. I/We agree to return uniforms and other equipment belonging to the "Penn Township Athletic Association" in as good as received condition, less reasonable normal usage.

3.    Injury Disclaimer 

PTAA is not responsible for monitoring the health or injury status of your child. PTAA is not responsible for any injuries incurred by your child during his or her participation in PTAA activities. As a parent, if you think your child (defined herein as under 18 years of age) may have an injury, preexisting or otherwise, you should: 1. Remove your child from play. 2. Keep your child out of play on the day of the injury. Your child should be seen by a healthcare provider and only return to play with permission from a healthcare provider who is experienced in evaluating injuries. 3. Ask your child’s healthcare provider for written instructions on helping your child return to play. You can give the return-to-play instructions to your child’s coach. Do not try to judge the severity of the injury yourself. Only a healthcare provider should assess a child for a possible severe injury. You may not know how serious the injury is at first, and some symptoms may not show up for hours or days. A child’s return to sports should be a gradual process that is carefully managed and monitored by a healthcare provider.
